About Us

​

Healing Hearts (HH) is a Concurrent Supportive Housing Program.   HH provides community-based counselling and program services, in-house life management support within a supportive housing environment.

Our facility is semi-independent and is accountable and responsible for adhering to their Bail Release Conditions & HH rules.  If a client does not adhere to their conditions or to HH rules, the breach of rules/ conditions is reported to the client's assigned officer. 

 

We meet with clients individually, based on the needs expressed by the clients, HH orchestrator community-based referrals to the appropriate providers. 

 

It is designed for persons who have made the choice to eliminate or change their alcohol / drug use in order to live a healthier life, free from the harm of substance use and/or addiction.

 

For those who choose not to be totally free from drugs or alcohol, we offer harm reduction Housing. HH harm reduction center's on hard-to-place people experiencing homelessness, chronic concurrent mental health and substance dependence into supportive independent permanent housing.  

 

HH providing additional support and services as needed.  HH basic underlying principle is that people are better able to move forward with their lives if they are first housed. Housing is provided first and then supports are provided. 

 

HH delivers service from a harm reduction philosophy that includes, but does not require, abstinence as the ultimate form of harm reduction.

 

The program is committed to delivering services from a person-entered approach that is responsive to the unique strengths and needs of each individual.

 

The program strives to improve the health and social outcomes of people with concurrent mental health issues by providing stable housing and individualized support, based on identified goals related to each person’s vision of a happy, hopeful life.

 

Our Approach to Supportive Housing

 

HH Program uses a flexible and responsive approach to assist individuals who are homeless, at risk of becoming homeless, or who are considered inadequately housed, by helping them to secure and maintain housing. It addresses addiction, concurrent mental health issues, and other issues that may impact quality of life.

 

As an organization, our services are diverse; we provide emergency, transitional, and permanent housing and support services to low-income individuals and families, seniors, chronic and hard-to-place individuals with concurrent mental health disorders and addictions, individuals experiencing homelessness, life challenges or displacement.  

 

Individuals living in a HH home, learn or relearn values and responsible behavior and, slowly but surely, develop long-term behavior to ensure comfortable life. 

 

Such support services include, not limited to:

- Furnished home (Apartment Style Units)

- Life-Skills Development

- Detox support & Treatment referrals

- Community counselling and programs referral based on client's needs. 

- Access to daily Prescribing Pharmacist. HH works directly with a community pharmacy who is a prescribing pharmacist.

- HH has created a partnership with a local, walking distance Medical Center which works closely with our clients to deliver and administer medical services and psychiatric referrals.

- Mental Health Advocacy

- Legal & Child Welfare Advocacy 

- Assists with Legal and Court matters such as supporting Bail Orders for treatment and supervision while on bail.

- Assistance in Obtaining Mainstream Benefits, such as Income Support, CPP, CPP-Disability, Filing Yearly Taxes, Assisting with Immigration Papers.

 

Supportive Housing Placement

Housing placement in transitional or longer-term units will be offered based on the participant’s identified substance use goals. Transitional housing offers abstinence-focused support, while longer-term units can support individuals seeking any kind of harm reduction (including abstinence). 

 

Length of Stay: 

Some individuals live in HH homes a few months; others for many years. There is no time frame for how long an individual can access our services.  As long there is a need, we will be there. 

 

Eligibility Requirements

In order to be eligible, individuals:

·       Be at least 18 years of age.

·       Be committed to active participation in community-based counselling and support services and programs, residential treatment and other identifying services.

·       Be committed to follow HH rules and regulations.

·       able to live and complete activities of daily living semi-independently

We provide on-site life skills training for nutrients awareness budgeting skills and cooking skills.  Skill development consist of nutrition knowledge, monthly food budgeting, meals planning, preparation and meal cooking.
